Mr. Consumer, and his children came to our office on 6-29-11 and took advantage of our free New Patient Exam. This exam does not include free cleanings, only free exams and x-rays, and it has never been advertised as such. Cleanings for children...

are always offered to the parent. They can chose to let their children have the cleanings or not. They are under no obligations to do so. The children did have cleanings and fluoride treatments. These cleanings, which cost 172.00 total, were submitted to Mr. Consumers insurance, which he gave to us. Mr. Consumer's insurance did require a 25.00 deductible for each patient. This is clearing stated in his benefits package that was sent to us by his insurance carrier, [redacted]. We have a copy on file which I will provide. Mr. Consumer's insurance paid 69.00 for one , and 57.00 for the other. This left a total of 46.00 to be paid which was part of his 50 dollar deductible. The account has been accruing interest since then and that is why it is 94.25 as of today.
Mr. Consumer has not paid the balance. Mr. Consumer read and signed two disclosure forms regarding deductibles and insurance. I do not have any correspondence from him disputing the charges or asking us to dissolve this charge.

Secondly, we have never sent or reported Mr. Consumer to the Credit Bureau. It is not our practice to report such a minor amount.
The complaints of false billing charges, refusal to remove the charge, and false reporting to the credit bureau is unfounded.
