To Whom This May Concern, In regards to Mrs*** *** ID#*** she did have her consult with our office on
March 21, 2016.Mrs*** desired to use her established periodontist for her dental deviceTypically our office would send out a referral to Dr*** for an oral appliance and everything would be handled thru his office, In this case we would need to hear from Mrs*** or from her periodontist to know exactly how the letter for medical necessity needs to be draftedOral appliances require not only a prescription but also a letter of medical necessity because an authorization is required from the insurance to approve coverage for the medical applianceInsurances companies are very particular on how the letter is wordedOur office uses Dr*** for all dental appliances, that is why our office staff assumed we would be getting a letter request from his officeHence the reason for the delayThere clearly was some miscommunication between our office, Mrs***'s periodontist and Mrs***, nothing that was done intentionally or maliciously*** did reach out to Mrs*** and apologized for the confusionShe drafted a letter for her but more than likely Mrs*** will need another letter more specific to her needs once they try to go thru her insuranceIf there is any other questions or concerns please feel free to contact our office
