September 2, 2015Dear [redacted] This letter is in response to the letter we received regarding the above listed complaint number.I have reviewed the account for [redacted] This patient was seen by our physicians in the emergency department at Brandon Regional Hospital on 1/23/ [redacted] was billed for this service and processed this claim 3/6/as follows:Billed Amount $ [redacted] Allowed - $ [redacted] Paid $[redacted] Applied to Deductible $(Pt Responsibility) [redacted] was sent a bill for the balance of $on 3/24/24, 4/25/and 5/28/To date, [redacted] has not been in contact with our officeThis account was turned over to a collection agency 7/3/2014.After reviewing this account it is determined that this bill is correctAlthough, the physicians are not participating providers with Humana, we have agreed to accept the [redacted] allowed amountThe patient is still responsible for paying any deductible, copay or co-insurance as per their insurance policy.As a courtesy, I have requested this be removed from the patient’s credit reportPlease feel free to contact me at ###-###-####, ext [redacted] if you should have any further questions or need additional information.Sincerely,Corinne K.Supervisor of Patient Services

To date, [redacted] has not been in contact with our office. This account was turned over to a collection agency 7/3/2014.After reviewing this account it is determined that this bill is correct.
