[redacted] cancelled her appointment the day of her surgery resulting in our office unable to fill the appointment time Per our contract that all our patients sign it states that surgery appointments that are cancelled less than hours are subject to a $per hour cancellation fee Her appointment time was ½ hours long but she was only charged for the ½ hours of surgery time instead of ½ hours for the full length of the appointment To address her statement of not knowing that the crown was not included we discuss this at the exam that we place the implant and she is to return to her dentist for the crown On the financial that she was given it clearly states, “DDS to fabricate the definitive restoration and final abutment with the TBD in the fee and balance columns” the TBD is placed in these columns as we do not know the fees her dentist will charge fee this services If we can be of any further assistance is clarifying this issue further please feel free to contact our office directly at 206-575-1086.Thank you,Jessica R [redacted] Office managerPacific Northwest Periodontics206-575-
