[redacted] Dds and staff refuses to acknowledge my FL Healthcare Plus dental eligibility for services and refuses to refund my $228.00/01/2013.
Dr. [redacted] just recently acknowledged my approval for dental work/upper partial in 06/2013. I was approved since 01/2013 and didn't receive a phone call/notification of it.
Now Dr. [redacted] refuses to schedule an appointment for another broken front tooth in which x-rays have already been done.
I have presented my FL Plus Healthcare membership card. I have inquired customer services and requested an eligibility form be faxed to Dr. [redacted] Dds for needed services.
I have also informed [redacted] @ Dr. [redacted]'s office of all of this yet she refuses to acknowledge or respect my rights for services, refund, etc.,
They are also refusing to return my calls in addition to scheduling an appointment...
