I went for an eye appointment and had the eye exam performed. Dr. *** did an extra test and did not explain to me that it would be an additional cost outside of the exam cost. They charged an extra $75.83 and did not explain that it would not be part of my eye exam and that I would have to pay additional fees for the test. I did not even request the test. I thought it was all part of the routine exam. She did not explain it would be additional and did not ask my permission or explain that it was not included in the eye exam.
We phoned the patient after receiving your letter and left a message regarding this matter. The patient did sign a form allowing us to bill his medical insurance, which we have enclosed, the original is on file. There was a medical diagnosis and this was discussed during the exam. We reversed the claim end wrote off his deductible this time.
