I am in receipt of your letter dated December 2, 2015 indicating that Mr. [redacted] had some concerns regarding...

scheduling his two final dental appointments at Eastman Institute for Oral Health. Additionally, he relayed that he has not received a bill since August of 2015. Mr. [redacted] listed as a desired settlement an adjusted bill and appointments to complete all necessary treatment. Please know that I have thoroughly investigated his concerns and would like to share some preliminary findings with you. I noticed that his last appointment was November 9, 2015 and he has yet to receive a bill. It is usual and customary for us to bill on a monthly basis. Unfortunately, his visit coincided too close to this date so it was bumped to the next cycle. Mr. [redacted] should receive an itemized bill on or after December 11, 2015. I have asked the manager in the Prosthodontic Division to contact Mr. [redacted] to schedule these appointments. If any adjustments are to be made to his bill, I recommend he work with the Division Manager to rectify. I would like to apologize on behalf of Eastman Institute for Oral Health for an inconvenience and misunderstandings that have occurred.
