*** *** *** enrolled *** for a club membership on 10/17/ In an email dated 6/18/she provided written authorization to EFT the bank account monthly for the dues for this membership The email stated "I authorize the amount of $to be charged on the 15th of
each month until notice is received to cancel membership". Annette K***, the club manager / owner FIRST heard of the request to cancel in a phone conversation *** and she shared on 5/31/ This is the first and only time the club has been informed that *** requests the membership to be cancelled Annette informed her during that phone call that the membership needed to be cancelled in writing at the club site however as she herself was the manager of the club she would put through a cancellation form today on ***'s behalf (5/31/17) There was no conversation about a refund of dues There was no prior incident or attempt to cancel the account.
