We haven't received death certificate and funeral was [redacted]Our dad passed on [redacted] We followed-up with EH Ford two weeks after the death but haven't received the death certificate. I called MS vital records and they haven't received the order as of [redacted] We have been given the run around with every call to EH Ford. We were told that [redacted] records was causing the delay but that wasn't the case. We ordered copies of the death certificate ourselves. Desired SettlementIf they receive the death certificates, please mail to address on file. If they haven't ordered them, reimburse charges for death certificates. Business Response Contact Name and Title: [redacted], Office MgrContact Phone: (XXX)XXX-XXXXContact Email: [redacted]@aol.comDear Sir or Madam: Please accept this letter as our response to the above referenced complaint. It has always been the policy of E.H. Ford Mortuary to help and aid families during the loss of a loved one. It is always our intention for the process of making final arrangements to be handled in a smooth and professional manner. We are truly concerned and deeply sorry if the [redacted] family felt as if they were given the run around pertaining to their father's death certificate. However, as we explained to the family, each state has their own protocol, procedures, and turnaround time for death certificates. Funeral homes have no control over the turnaround time for death certificates. Nor do we have the ability to rush the doctor or attending physician who signs the death certificate. In the state of [redacted] the initial death certificate is generated in the city where the death occurred, then sent to the funeral home so that the vital statistic data can be added. Then the funeral home has to file the final copy of the certificate in [redacted] where all certificates are filed for that state. When making arrangements, the process for that particular state is explained to our families as we did with the [redacted] family. For clarity and to provide an explanation, listed below is the timeline and process that occurred in obtaining Mr. [redacted]'s death certificate:Mr. [redacted] passed ** February [redacted] at [redacted] in [redacted] The [redacted] family made arrangements on the following day, the [redacted] The death certificate might have been signed by the doctor that day, but we did not receive it until a few weeks following his death. A call was placed to Medical Records at [redacted] in [redacted] on February [redacted] because we had not yet received Mr. [redacted]'s death certificate. Medical records advised that we were listed as the receiving funeral home and they confirmed our address. They also stated that the death certificate would be placed in the mail. After a few days passed by, another call was made to Medical Records on the [redacted] of February, and we were advised that the certificate had been mailed. Also on the [redacted] called to add an additional 7 death certificates for a total of 10 certified copies. Ms. [redacted] was advised that we were still waiting to receive the death certificate but we would get the additional certified certificates as requested and she could pay for the additional copies later. She was also advised that the certificate would have to be filed in [redacted] upon receipt. We also advised her that we would contact the family once we received the certified copies. On February [redacted] we finally received Mr. [redacted]'s death certificate from [redacted] We then completed the certificate with the vital statistic information provided by the family and mailed the final copy to [redacted] Vital Records along with 3 other certificates we had to file in [redacted].On March [redacted] we called [redacted] Vital Records to check on the status of all of our death certificates and the clerk informed us that they were two weeks behind and asked us what funeral home we were calling from and the certificates we were inquiring about. After stating our firm's name and Mr. [redacted]'s name, the clerk asked us not to call anymore as they had received several calls about this certificate. She also stated that as a funeral home we should know the process for filing certificates and to let the certificates go through the process. On March [redacted] another call was placed to [redacted] Vital Records and we were told that the death certificate had been filed and that there were two requests made for certificates. One request was by the funeral home and the other one by the family. On March [redacted] we received the certified copies from [redacted] Vital Records. We sent the requested 10 copies overnight via [redacted] (at no additional charge) to the informant, [redacted] Mr. [redacted]'s daughter. Additionally, we sent a refund of $25.00, the amount paid during the arrangement conference for 3 certified copies. Ms. [redacted] stated in her complaint that an order for certified death certificates was not placed with [redacted] Vital Records on March [redacted] Please be aware and understand that because she could not place an order, does not mean [redacted] Vital Records did not have the final copy. Until the final copy was processed by [redacted] Vital Records, no one's request for certified copies could be completed. In conclusion, the state of [redacted] has its own process for filing death certificates as does every other state. While the process can be lengthy and frustrating to families and funeral homes, it is still a process that we cannot control. We can only communicate with hospitals, doctors, and vital records and document as we did with Mr. [redacted]'s case. If we could have done more, we would have. We apologize for any misunderstandings and miscommunications between our firm and the [redacted] family. It is our prayer that an explanation of the process will resolve this matter in a satisfactory manner, and we regret any inconvenience this may have caused the [redacted] family. Respectfully Submitted,[redacted]Office Manager
