To [redacted],Please review the attached communication log notes directly from [redacted]'s electronic health record.  In summary; it is noted that her Medical Assistance Insurance termed on March 31, 2017 due to her income level.  Since Ms. [redacted] no longer had insurance and...

indicated she was unable to pay out of pocket for her dentures to be made in April; the appointments were cancelled.  Ms. [redacted] requested that we bill for the dentures in March since that is when her insurance was still active.It is unlawful for Apple Tree Dental  to bill Insurance for procedures that have not yet been completed. Please follow up with the complainant accordingly to inform her that we are more than happy to make her a set of dentures; however she will need to pay for them or wait until she is eligible for Medical Assistance again.Thank you and please contact me if you require any additional information. Karen M.
