First of all, at the cancellation of the appointment in the office, I leftThe clerk is lying when she says it was explained to me about a security container (if that was the case, I might not have been so concerned about my information being in their office), at NO TIME DID ANYONE MENTION A LOCKED SECURITY CONTAINER.It was later in the evening that I thought about my information being at this office and I called, I never got an answerI started calling early the next morningAfter identifying myself I asked to speak with Rosa, the lady I had talked with the day before, at first they tried to give me the run around, she (Rosa) was helping someone and would call me back, I told her I would holdWhen Rosa came to the phone I told her who I was and that I wanted my information returned, she hemmed and hawed and says that the information had been entered into the computer and they did not keep the original papers [redacted] NEVER SAID ANYTHING ABOUT PAPERS BEING ENTERED INTO A LOCKED SECURITY CONTAINER BUT SHE DID SAY THEY WERE SHREDDEDI just don't believe they were ever shredded.I will continue be leery of this office and their proceduresI will also continue to check my credit report for any suspicious activity

Patient made her appointment over phone on 11/20/2015 she did not provide insurance information. Patient then came in for her appointment on 12/1/2015, she then presented with her driver's license and her [redacted] insurance card and was given a registration form to fill out while her demographics and insurance information was being entered into our EMR (electronic medical records).
Her insurance card, driver's license and her registration form was scanned into our EMR, her cards were given back to her and her registration form was put into our security box for shredding after it was scanned.

The clerk is lying when she says it was explained to me about a security container (if that was the case, I might not have been so concerned about my information being in their office), at NO TIME DID ANYONE MENTION A LOCKED SECURITY CONTAINER.It was later in the evening that I thought about my information being at this office and I called, I never got an answer. I started calling early the next morning. After identifying myself I asked to speak with Rosa, the lady I had talked with the day before,
at first they tried to give me the run around, she (Rosa) was helping someone and would call me back, I told her I would hold. When Rosa came to the phone I told her who I was and that I wanted my information returned, she hemmed and hawed and says that the information had been entered into the computer and they did not keep the original papers.
