Mr [redacted] *** [redacted] came to our office on July 16, for hearing exam and testsHe purchased hearing devicesHe told us to file his Blue Cross Blue Shield Insurance and signed the proper documents to authorize us to do soHe also signed a contract that stated he was NOT entitled to a refundOnce Insurance claims are filedAll sales are finalThis was signed on July 16, I have attached a copyThereafter he has come in for adjustments and remake of hearing aid to ensure proper fitHe came into the office on 08/14/and left the custom hearing aids on the counter saying he was not going to wear themHe did not dispute the charge until 09/08/These hearing aids belong to Mr [redacted] and are under warranty for yearsIf there is a problem with the sound and proper use we will be happy to help with this matterNo insurance claim has been paid for this patient

Mr. [redacted]  [redacted] came to our office on July 16, 2014 for hearing exam and tests. He purchased 2 hearing devices. He told us to file his Blue Cross Blue Shield Insurance and signed the proper documents to authorize us to do so. He also signed a contract that stated he was NOT...

entitled to a refund. Once Insurance claims are filed. All sales are final. This was signed on July 16, 2014. I have attached a copy. Thereafter he has come in for adjustments and remake of hearing aid to ensure proper fit. He came into the office on 08/14/14 and left the custom hearing aids on the counter saying he was not going to wear them. He did not dispute the charge until 09/08/14. These hearing aids belong to Mr. [redacted] and are under warranty for 2 years. If there is a problem with the sound and proper use we will be happy to help with this matter. No insurance claim has been paid for this patient.
